Question about filling out G-325A

A few questions...

1. The G-325A form from the government website says Current Husband or Wife (If none, so state) Family Name (For wife, give maiden name) in one box, where as the sample form has two seperate boxes, which one is correct and how do you fill out the Gov. supplied one?

2. The form asks for "Applicant's residence last five years. List present address first.", then right under that asks for "Applicant's last address outside the United States of more than 1 year.", are they just asking for the most recent address? The one they will have lived in up until leaving?

3. In the part that says this "Applicant: Print your name and Alien Registration Number in the box outlined by heavy border below.", does that have to be written by hand or only the signature?

Thanks to all those who help, it is greatly greatly appreciated. Kind regards

Here are the two forms I am talking about...

US GOVT: http://www.uscis.gov/files/form/g-325a.pdf

Sample from third party: http://tinyurl.com/sample325

1. Maiden name for woman or family name for man goes in first box.

2. Means what it says. Sometimes the most recent address is for less than a year. For your own G325a the answer may be "NONE"

3. Type or hand print is fine.

Thanks, but how do I write my Current Wife and my Family name in the same box? The form from the USCIS website has both those things written above one box.

Your current wife is not a former wife. The lower box asks about former spouses. For a wife or former wife it is the maiden name that goes in either box. Your name has nothing to do with anything. On the petition itself, if your wife changed her family name to match yours, you'll put her new current family name on her side of the I-130 and other forms that ask for her current name. Stop thinking of it as your name. One place asks for her maiden name and one asks for her name in the present tense. For my wife who never changed her family name the answer is the same. For your wife, it sounds like she has a maiden name and a current married name. (NOT your name but HER married name.) If she had former marriages where she used other family names, you indicate that were asked as well.

So this "Current Husband or Wife (If none, so state) Family Name (For wife, give maiden name)" is asking for her maiden name? Thanks for the explanation. They word it so it looks like two separate questions

Yes, for current wife, they want her maiden name and then first name in that space on the G325a.

this brought up another question...my wife is from Colombia and it is a VERY difficult process to change your name in Colombia so most women do not change it. My wife DOES want my last name when she gets to the US for all purposes, so would I put my last name or her maiden name on all documents for the CR 1? She does want my last name, its just that Colombia makes it extremely difficult on people and she won't ever change her name in Colombia.

The name must match whatever is on her passport. Even if you do put her married name on the documents, the visa/GC will be issued in that name.

When asked for maiden name, give maiden name. When asked for "name" give current legal name. Obviously in your wife's situation, unless she's been married and changed her name once already, her current legal name and maiden name are the same. "Your" name is used only when questions about YOU are asked. When and if your wife changes her current legal name, what you are referring to as "your" name would then be "her name".

got ya...so I guess that means all the documents would be under her maiden name, and once she changes her maiden name to her married last name, we have to go and fix all the documents...including her green card and social security card?
