Medical in the morning, need help fast.

Hey guys,

I know I'm leaving it to the last min, my medical is in the morning in Brisbane Australia, I remember reading somewhere that it's a good idea after the medical to ask the doctor for........(this bit I can't remember) a form of some kind, maybe even a print out of what needles you had. Something to do with it helping lated in the process. when you change your statues after your married. Can anyone help, what am I meant to ask for? or I have read so much over the last few months that I'm confused?

What vaccinations are required?

The following vaccinations are required for immigration purposes:

Hepatitis A

Hepatitis B

Influenza

Influenza type b (Hib)

Measles

Meningococcal

Mumps

Penumococcal

Pertussis

Polio

Rotavirus

Rubella

Tetanus and diphtheria toxoids

Varicella

You are going to ask him for a copy of the vaccination sheet, form DS-3025. This is where he writes your vaccination history and at the bottom of the form it says "give copy to applicant". Also ask if it is possible for you to get a copy of the ENTIRE medical for yourself (this doesn't always happen because he doesn't HAVE to) because there have been some issues with USCIS losing the medical (well not really, just too lazy to look for it) and this will save you time later.

I called my doctor from America to request a copy of my medical and my mother will forward it to me. I told him I didn't mind if it was in a sealed envelope marked "COPY" and "For USCIS only" but that in case I get told they lost my medical I have a copy to send them.

Nope. Completely and utterly irrelevant to Australia.

The OP simply needs to ask for DS-3025 - Vaccination History which is part of the medical paperwork. In Australia we take our vaccination records (whether it be from our baby book, or a blood titre, or evidence from a doctor) and it is transcribed onto that form. The doctor will then assess whether other vaccinations are needed (like he made me get Pertussis even though I've HAD it I didn't know which type it was and didn't want to waste time waiting for a titre). If none are needed he'll mark the form as such and send you on your merry way.

He will then send the completed documents to you (I think this is all states, either that or straight to USCIS). You should get the Xray, a sealed envelope marked "USCIS only" (I think actually not all states do the "sealed envelope thing") and a copy of the vaccination history. If the OP is one of the lucky ones who's medical isn't sealed then they can make a copy of it all themselves. This will save a lot of effort later (esp if they get an RFE during AOS for the copy of the medical).

In my case he didn't send the copy immunisation sheet with the medical stuff he sent to me (and I forgot to remind them and my entire medical was sealed) so I called once i received the medical and noticed it wasn't there and had them send me the vaccination record. There was no problem because it says to give it to me at the bottom of the form.

This is great information to have. I think this applies to all Countries and all Consulates or Embassies because ultimately the information is to be used in the US for AOS.

We do have this, too. It is yellow and it's free. Must be a universal thing. I already had one and she just added the ones I had for my immigration. Nothing to do with immigration over here, though. But still a great way to keep track of what I've had because I have no vaccination records apart from that.

Were you born in Australia? I was, and the only "booklet" I have is from birth but it's not a "international vaccination record" it's a "baby book". Where you write my length, weight etc, immunisations, any time I get sick or break an arm/leg.. but it's written by the parents (except the vaccination part) but even then the vaccination section isn't signed in any official way... It's only valid in Australia BECAUSE it's not signed in any official way and BECAUSE the parents keep it and change it at will.

There is this: http://www.travelclinic.com.au/pocket-guide.asp but it appears to be some sort of private company thing (i.e not an Australian standard) and I've never seen or heard of it before. Wouldn't have known it existed if I didn't google it out of interest. I might get one considering it's recognised by "WHO" but it says I need to have a visit with them... umm... maybe when I'm in Aus next? or hopefully I can get around that...

Yea, it's a great little booklet. I wasn't born here and the first time it was offered to me was a couple of years ago from my local GP. It's a little traveller's booklet. Definitely if you're heading back this way, take your records to your GP (if they do travel vaccinations)and ask them for it and they will transcribe it for you. I'm at work or I would have scanned it for you and posted a pic! :lol:

Hey, just so you know, Dr Yates will give you the medical unsealed (unless there is a massive problem with your results) so you can actually make a full photocopy of your entire medical packet before you send it off to Sydney...I did that and I have two additional copies of all the paperwork and results in case I need it. Pretty good to have.

Hope the medical went well..I have to say I was freaked out a little bit by the fact I was undressed for the whole medical, and the fact that he wouldn't listen when I told him he wouldn't get a vein in my right arm but tried for about five minutes anyway...apart from that the receptionists are very nice, and so are the people where you get your xrays done.

Oh, another thing....you may get a pathology bill from Sullivan Nicolaides for your blood work....DO NOT PAY IT and just let the Doctors office know that they tried to bill you. You already pay for all the xrays and bloods at Anzac Square, so they bill might be sent to you by accident.
