Timeline

Since you returned your green card at the border when you returned to Canada I don't think this form will apply to you.

(1) You had the status of an alien lawfully admitted for permanent residence at the time of departure from the United States;

(2) You departed from the United States with the intention of returning and you have not abandoned this intention; and

(3) You are returning to the United States from a temporary visit abroad and, if the stay abroad was protracted, this was caused by reasons

beyond your control and for which you are not responsible.

I would call USCIS and ask them if you are in the system as having abandoned your residency. If this is the case (since you returned your green card at the border) I'm not sure this form would apply.

It would help people who want to answer you if you posted some more of your information in your thread. Then we can tell if this applys to you or not.

As per your other posts it seems you gave up your LPR at the border when you gave away your green card and said you wouldn't be returning because you can't get used to living in america. The DS-117 appears to be for people who still have their green cards/proof of LPR status. Which you gave up on returning to canada.

In your other threads many people have already also stated you need to refile from the start.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

Yes, I think you right. The form does state medical reason, but I don't think it will apply for me. You can't trade a green card for a medicare. I will have my husband call them. I do believe we have to start over again with the 1-130. We married in 2007. Here we go again. This time IF, God gives me the chance of going to the USA, I will get the flu shot, once a year. This is going to be costly for me.

Yup, will need to file a new I-130.

Probably a great time to review the Guides section here, email the links to your husband in the USA (since he's filling out a new I-130 petition for you)
