Long Form Birth Certificate

So, apparently I do not have a long-form birth certificate. Does anyone have an advice how I should go about registering for one? I have the short-form ones, but that's all and I hear I'll need a long-form one for the later part of the K-1 process.

I live in Ontario and all I did was google "Ontario long form birth certificate" and it popped up right away

Ontario, as well, though all I seemed to be able to find was if I was registering a birth certificate for a new child.

Google "Office of the registrar general" for Ontario.

You can request the birth certificate online and it will be mailed to you. I applied from here in the US to get one for my son. It made it here in about 2 weeks.

I was adopted at the age of 6 weeks. Do I request the name I was born with or the name I was given when I was adopted? My current birth certificate has my adopted/present name. But as I was reading the form online it wants to know mother's & father's names. Do I put my birth parents names or my adopted parents names?

Put your adopted parents names. Apparently when the adoption goes through your old birth records are sealed, and documentation is redrawn with the names of your adoptive parents. It's very strange when they want to know things like how old your parents were when you were born, but you just go through the motions and write it in like they were your parents at birth.

I was adopted by my father and my name changed as a child, so I'm speaking from personal experience

I've been reading about long form birth certificates but the ones I got from Italy are really just slightly longer than an 8x10/11 sheet. That's all they give you so I'm hoping this won't be an issue....it's as good as it gets.

Here's a direct link to the Ontario Birth Certificate online application. Read it and click the button at the bottom. Flames9_RN, it wasn't completely clear from your links that you can apply directly through your browser, without having to mail anything out.

I did this at little more than a year ago and got it within a few days (I paid for expedited processing).

If it has your parent's names on it, it should be fine. That's primarily what the US/Canada/UK mean when they call one format "long form" vs. "short form" - the "short form" birth certificate lacks the parent's names.

The US Consulate in Italy should be able to give more localized advice on exactly which format to order from the Italian government.

This. Also, the Short Form is literally short. It's wallet size.

Remember that if any part of it is in Italian, it will need to get a certified translation, complete with an embossed seal.
