Cannot find fingerprint form (K-1 Visa US/Australia)

Please help if you have info or experience.

My Australian fiancee is preparing her Packet 3 from the US Consulate in Sydney. She called to make an appointment for fingerprinting. The constable (police officer) in Victoria insisted that she obtain a "fingerprint form" from the US Consulate to bring to the appointment. Neither of us can find the form anywhere, and we have not been able to speak to anyone from the consulate directly.

If you know where we can find this form (online), please respond.

That is incorrect, there is no such form as the one they are telling you to get. All you need to ask for is a national police check for both name and fingerprint. Don't even mention that it is for a visa, because thats when the Police get confused. You can also have the check sent directly to you house, there is no need for it to be sent directly to the Consulate. The check takes a few weeks to be processes (Mine took almost three weeks). If you read This Thread, the OP is from Victoria and I am pretty sure she has info about getting her check done. It also has general info about going from Packet Three to the Interview.
