From your timeline, you're waiting for the NVC to forward your petition to Manila. The embassy there will send your fiance/ee "packet 4" which has all the information that she needs to begin the visa application process. Manila doesn't seem to issue "packet 3" but includes the interview letter along with the instructions/ other forms.

At the moment, visa fee itself is $131, but it is supposed to be going up to $350 sometime soon. Depends on when your interview takes place.
