as long as it is:

1. filled out and

2. signed and

3. scanned in, converted to PDF format, it won't matter.

What does matter, though, is that OPT IN Email and the format.

The addresses that WERE in there, are now (as of today;) ) obsolete.

Cool ! You should hear something back over the weekend (SERCO contractors work on the weekends, they are crazy strange on workloads) - unless NVC is shut down for the weekend from the weather.

2009Married, if the old email address no longer exist then you should receive a "delivery failed" form your mail server. But send it again to the up-to-date email won't hurt.
