For Question 10, I did the following in the space provided for the Name of Person:

Jane Doe I129F Case Number HCMxxxxxxxxx

Relationship - Fiancee

Date Submitted - I entered the date from the NOA1

For Question 11 I checked neither box, in the space provided I entered:

N/A (K-1 visa process for permanent residence)

As far as taxes - I save a copy of my online filed tax returns and that is what I sent. I did sign and date them with the date that I filed them online.

We had no problems and no questions.

You can call the IRS and request for them to send you the tax transcripts for whatever years you want. I called and got three years worth of mine and my mother's in about a week.
