USCIS ACCREDITED CLINIC/SURGEON FOR THE SHOTS BEFORE AOS

hi everyone..id like to know the link or how to find the accredited clinic/surgeon of USCIS for my additional vaccination/shots. Presently im in Waukesha, Wisconsin. I need a Tdap and Varicella(MMR) shots before my AOS. I was told at St. Lukes in the Phils during my medical that Ill have 2 more shots here in the US after 2 months which is on Feb. 1, 2010 but I dont know where to find that clinic here..and also how much would it cost us for those shots??

hi, dai! how are you? if only for your shots completion, i suggest you first look for the nearest health department clinic in your area. i did that and it worked well for me. didnt cost me anything because of our health insurance but if you will have to pay, i think it's only 12 dollars per shot/visit.

Removal of conditions Journey

16 March 2012 Sent I-751 package from Aviano AB, Italy.

29 March 2012 Received everything back...wrong fee. thought we didn't have to pay biometrics since we were sending fingerprint cards and passport photos.

30 March 2012 Sent everything out again from Aviano AB, Italy.

10 April 2012 Check cashed

17 April 2012 Received NOA1 dated 6 April.

06 Dec 2012 Received 10 yr green card. Letter said it was approved 28 November 2012.

You can get the shots from a local doctor, clinic or health dep. Save the paperwork and take it to the CS when you find one.

Shots don't have to be done by the CS.

Hope they all don't try and rip you off by trying to do a complete medical.

Shouldn't cost more than $50 for the vaccination record transfer.

hi there dakine..thanks for the tip..my question is--it will surely save us $$$ if i go to a local health dept., clinic or doctor for my shots and not to the USCIS accredited CS, but would they issue me n I-693 vaccination sheet in sealed envelope?? or will i just get the shots from them and then proceed to the USCIS CS for the I-693? well, i dont wanna get naked again for the physical exam like what my friend experienced in LO..i just need the shots, u know what i mean...sorry for all the questions....and thanks btw for the helpful reply

As Dakine told you, you can get the shots anywhere. You need to take proof of the shots to the Civil Surgeon to get the I-693 transcribed. The I-693 can only be transcribed by an authorized Civil Surgeon.

And as also mentioned, many Civil Surgeons will try to sell you the complete physical to get the I-693 transcribed. So you may have to call many civil surgeons or take the directions to them.

04/29/14 - biometrics date

07/01/14 - interview date

Remember to also take the papers the nurse gave you at St. Lukes.

The ones that listed all the shots you got at that time.

The CS will need that paper to complete the I-693
