hello,

Kind a confused here, need your advise...........

what to write on PART 3, SECOND PAGE, question number 1.

spouse didnt change her last name to the husband last name

SECOND QUESTION,

as we moved from our last address, on question number 9 on SECOND PAGE..............if i answer YES, then it says, ATTACH A LIST OF ALL ADDRESSES AND DATE..........how to attach this................do we just write on blank peace of paper FROM and TO dates and the complete address.................please correct me if i am wrong...............I HAVE A COPY OF CHANGE OF ADDRESS that we did online and AR-11 CHANGE OF ADDRESS FORM

THIRD QUESTION,

is photograph and affadivit from friends is the main documents that we have to include or we can skip this and put rest of the documents that we have collected.

4TH QUESTION,

is PASSPORT SYTLE PHOTOS REQUIRED

if she never changed her name to husbands name just use the maiden name ,

The application has to be in the name she uses after marriage

( if she kept her maiden name then aply with maiden name ), other names used is maiden name or any other names she uses

as we moved from our last address, on question number 9 on SECOND PAGE..............if i answer YES, then it says, ATTACH A LIST OF ALL ADDRESSES AND DATE..........how to attach this................do we just write on blank peace of paper FROM and TO dates and the complete address.................please correct me if i am wrong...............I HAVE A COPY OF CHANGE OF ADDRESS that we did online and AR-11 CHANGE OF ADDRESS FORM

yes write all the info on a blank papper add copies of what ever prove you have papperclip it to the back of the application

THIRD QUESTION,

is photograph and affadivit from friends is the main documents that we have to include or we can skip this and put rest of the documents that we have collected.

main prove would be joint tax return , joint bank accounts , joint lease or mortgage uttility bills

anything financuel that has both your names on it

but yes pictures and affidavits can prove alot if you dont have enough other prove

4TH QUESTION,

is PASSPORT SYTLE PHOTOS REQUIRED

No they take the passport style photo at the biometrics

April 14/2017 send N400 

04/25/17 credit card charged 

04/25/17 e mail NOA send 

05/01/17 hard copy of NOA dated 04/25 received in mail

05/06/17 biometric hard copy in mail 

05/19/17 Biometric appointment in Hartford CT 

07/17/17 Inline for Interview 

07/24/17 Interview letter in mail 

08/24/17 Interview in Springfield MA ... Yes Aproved

09/14/17 Oath Ceremony .... done I am a US citizen

09/22/17 Applied for Passport ( per reg mail ) 

10/04/17 got passport in mail  

10/13/17 got certificate in mail  , updated status with social security office 

AM DONE YEAHHHHHHHHHHH

No. USCIS lists these documents as examples, and not requirements. It never hurts to include them if you have them handy.

Just remember if you include affidavits, they need to be originals.

4TH QUESTION,

is PASSPORT SYTLE PHOTOS REQUIRED

NO. They will take your photo when you go for biometrics.
