my questions has been already answered about where to put the last names, because there is no middle name. The problem is that in the bottom box she has already printed her name as if she has a middle name. On the part for signature she signed it correctly as her name is written ,it's the printed box with the heavy border that I am concerned. I filled out all of the paper for her and she signed, and also she signed blank G-325 A but with the same mistake. I can refill everthing out and put everything else the way it should be. Will that bottom box with the heavy border cause me problems ?

What do you mean "she already printed her name as if she has a middle name"? Specifics please.

I had her to sign her name on the signature part which was not a problem, but the bottom box with the heavy border , she printed her name last, first, and then middle but I just learned to night that it should be both last names together and then her first name and leave the middle name blank.

Answered in your other thread on this same issue. Enough already! She got her name in the box...relax.
