Is a copy of the medical report needed for AOS?

hmmm... she will need to have a valid medical exam in order to apply for AOS. If it has been less than a year since the medical exam in Moscow at the time that you submit the AOS application then the results of that exam are valid for the AOS. If more than a year has passed since the medical exam, then she will need to retake it.

The results of the medical exam are included in the big brown envelope that she will present when she arrives at the POE. It is a good idea to ask for a copy of the DS-3025 (the vaccination supplement) whenever she has her exam in Moscow.

Here is the actual info from the AOS Guide--

Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. You will, however, be required to complete portions of the I-693 and have a Civil Surgeon certify the form (and seal it in an evelope). Specifically you will complete Part 1 (Information about you) of the I-693 and provide both the form and your DS-3025 (if you have it, proving your vaccination history) and any proof of required vaccinations not completed prior to entering the US. If you do not have a copy of your DS-3025 you will need to provide sufficient evidence of your vaccination history to the Civil Surgeon (talk to the civil surgeon's office to discuss what vaccination records they accept as proof). If you do not have proof of your vaccination history the Civil Surgeon may insist on providing the required vaccinations prior to completing the I-693. Once the Civil Surgeon has has verified that all required vaccinations have been performed, the will complete form I-693 Part 2 (the vaccination section) and Part 5 (the Civil Surgeon's Certification). They will then seal the form in an envelope and return it to you.

You can find a list of Civil Surgeons on the USCIS web site. Shop around for costs since they can vary quite a bit. Not every medical doctor is certified to be a Civil Surgeon. I think it's a racket what they charge versus what they do.

But not all are that way---

Our first time through the process we went to the local Civil Surgeon. He was an elderly gentleman, a retired physician who now worked as a Civil Surgeon about 5-6 hours a week. We didn't need a full medical -- we only needed the vaccination info transcribed. I'm sitting in his office looking at the various signs posted around the place. One of them said something about ALL patients requiring a full medical exam.... another said something about the minimum charge being $200 dollars.... another said something about all charges having to be paid at the time of service. I'm sitting there thinking, "Oh my God, how much is this gonna cost me!!??" :o We finally get called to the back .... we go to his office and sit there talking with the guy for well over an hour. We talk about meeting people from abroad, the traditions of various countries, etc, etc. And as we're talking, a little voice in the back of my mind is saying, "Oh... now I get it--- this guy is gonna charge us by the minute!" Finally, he gets around to actually transcribing the vaccination records onto the form. We talk a little more as he explains a little about what to do with the form and a few stories about people who had encountered various problems in the AOS process.

We get up to leave and the wife asks him how much we owe him. He says, "Oh for that little bit of nothing that I did, I'm not gonna charge you anything." :wow: So-- I grabbed the wife and hurried out of there before the guy changed his mind.
