So, possibly ignorant question about to be asked... blamed on lack of coffee/sleep and the result of travel ordeals... :whistle:

Where was my passport issued? The issuing authority is listed as Scarborough... Scarborough, ON, I'm assuming (assuming, as it doesn't list it). I filed for the passport through the mail, way back when.. and now, on the DS-156 it's asking for City, Country, State/Prov of Issuance... and then still asks for Issuing Country in another box as well.

its whatever is listed under the authority...which you said was Scarborough, and then Canada for the country. I know after filling in so many forms simple questions make you second guess! lol Good luck with the rest of the process. Take care.
