hello to my fellow pinay vj members.. just want to ask something about this form.. ds-156k, says here in the lower part,

" DO NOT WRITE BELOW THIS LINE "

THE CO WILL ASSIST U IN ANSWERING THIS PART...

OF COURSE I UNDERSTAND IT... BUT I WHAT I READ HERE IS THAT... FILL UP ALL THE BLANKS.. DON'T LEAVE ANY BLANKS... WILL I REALLY NOT ANSWER THIS PART...?, OR I CAN WRITE MY FIANCE'S NAME ON IT AND SIGN IT ALREADY?...

the maiden name is when you're married. for example, you were smith, now jones. jones is your married name, smith is your maiden name (which you formerly went by). as you are a k-1, block 6 lists your current last name.

a surname is:

A name shared in common to identify the members of a family, as distinguished from each member's given name. Also called family name, last name.

1. do not sign in. you will sign in on your interview.

2. you're the fiancee (the one applying for visa), you are the one to sign it, not your USC fiance)

3. if you are single and a simple name ex. maria (first) santos(mother's last name in the phils/ middle name for americans) dela cruz (last name) and have never been called and used this name since you were born, you don't have anything else to put in "other names"

other names refer not only to other names used (if you were previously married) but also to aliases (if you used any at work/community etc) and third or fourth names (very common in the phils)

so if you have a long name ex maria (first) lourdes (second/other name) santos (middle name) dela cruz (last name)

in other words, whatever you are not able to include in the last name, middle name and first name category, indicate in "other names"

You can download the forms through the Department of State website: http://travel.gov.us Follow the links to get the forms. Pero they are not saveable.

If you want saveable DS-156, DS-156K and DS-157 forms, look for any posts made by Stephen + Elisha. He has made them saveable. :)

02-23-2018: Date on NOA, retrieved from online account

02-23-2018: Date on Biometrics Appointment Letter (Biometrics Appointment at Jacksonville ASC on March 13, 10:00 a.m.)

03-08-2018: Biometrics complete

04-05-2018: Case status updated - Interview Scheduled on May 10, 2018, 10:15 a.m. :D

05-10-2018: Citizenship Interview - Passed English and Civics Tests, Recommended for Approval! :D 

06-19-2018: Received email and text notification: Naturalization Ceremony Scheduled; waited for letter to be uploaded on online account - it has been set on Wednesday, July 25, 3:00 p.m.

07-25-2018: I am now a U.S. Citizen!

hello sis..i just had my interview yesterday..i didnt write anything on that part too since the instruction is very clear not to write anything...believe it or not...the woman that will give you your priority number will ask you sarcastically why you didnt sign that part..she will give your papers back to you and ask to fill it up...it will really waste your time coz she will not wait for you to finish it,,she will continue on giving numbers to the person next to you..so better sign it before you fall in line for the priority number.

01/21/09- (my b-day) started using instant messenger

02/17/09- decided to come in PI to meet me.

02/19/09- booked a flight and hotel

03/18/09- hired a lawyer for K1 Visa

03/25/09- sent G325A and other documents to love

03/30/09- love received the forms.

04/07/09- sent the forms G325A, I129F and other documents in checklist to our lawyer

04/09/09- lawyer received the forms and docs.

04/17/09- lawyer sent it

04/29/09- NOA1...case number received.

08/06/09- Love's 2nd trip....SOOO HAPPY..

08/14/09- Went back to US

08/26/09- NOA2 email

08/29/09- NOA2 hard copy

09/02/09- NVC letter received

09/22-23/09- Medical

09/29/09- Interview passed.........

10/14/09- Arrive in Texas

11/25/09- wedding day..

I NEED SOME HELP PLZZZZ

Two weeks ago i received the envelop from the us consulate in montreal, and they are saying : please complete and return to this office :

1-DS-230 PART 1

2-DS-160

3-DS-156

4-DS156K

AND THE CONFIRMING RECEIPT REGARDING FORM DS-160

AND CHECKLIST (ENCLOSED) INDICATING YOU HAVE OBTAINED THE REQUIRED DOCUMENTS

SO MY QUESTIONS ARE:

DO I HAVE TO SEND THEM ALL ONE SHUT, EVEN AFTER MY MEDICAL EXAM?

DO I HAVE TO SEND THE ds-230 FIRST AND AFTER THE REST?

SO PLZ IF SOMEONE CAN HELP ME, I AM AFRAID TO BE LATE IF I WAIT TILL I FINISH EVERYTHING AND THEN SEND THEM
