I have a question about form DS-156 for K3 visa. I will apply on July 19th and I'm really paranoid about filling out the forms.

The question 8: Other names used......

In my case, it's a little complicated. Me and my husband have a bank account in US on both names, my name on this account is my husband's last name, the lady at the bank changed it and asked only for the marriage certificate. All my other documents are on my maiden name, I didn't want to change anything because I was afraid not to mess it up, the petition was on my maiden name and everything else.

Now, should I declare this name from the bank account as "other name used", even if it's not a name shown on any ID?

I also used husband's last name when opening joint bank account (I thought I'll go by that name), but I ended up keeping everything else in maiden name.

The question "other names used" comes up often during the immigration process. I always put down husband's last name... and also used those bank statements as proof of relationship... and still have SSN and GC in maiden name. Nobody ever mentioned anything....

I have also been very confused by this issue. My legal name is still my maiden name, but I use my married name at work, etc. I just always put down my married name under "other names used" and it hasn't been a problem.

Also, I once asked a USCIS operator about this and they told me that the government assumes a wife will use her new husband's last name, so they almost expect to see this kind of thing if you haven't legally changed over your name yet.

10th Aug 2006 - NVC recieve IV Bill.

11th Aug 2006 - Recieved I-864 package / mailed back 15th Aug

I-129F:

6 Mar. 2006 - sent I-129F; 8 Mar. 2006 - I-129F received at USCIS

16 Mar. 2006 - NOA1

3 July 2006 - received RFE (IMBRA); 5 July 2006 - mailed RFE response

10 July 2006 - received USCIS email confirming receipt of RFE response

13 July 2006 - received RFE (IMBRA) AGAIN

17 July 2006 - mailed 2nd RFE response; received by USCIS 19 July

27 July 2006 - APPROVED!

7 Aug 2006 - case number assigned at NVC

11 Aug 2006 - Recieved NVC letter confirming case sent to london

23 Aug 2006 - Received Packet 3 / 25 Aug 2006 - Mailed back

04 Sep 2006 - Medical completed.

08 Sep 2006 - Interview date assigned for 2nd October

'other names used' is a very simply, straightforward phrase - yet so many people complicated by adding things that are not there. One thing not there is 'on any ID'.

You have used your married name, and I wouldn't be surprised if you've used it at places other than the bank.

Thanks a lot, your answers helped me a lot. My paranoia has an excuse, I've been away from my husband because of a former J1 visa with 2 year residence requirement, I just want to do it right and be back with my honey.

For Yodrak: I haven't used my married name at all, I'm still in my country, you cannot use a name here unless you have an ID (passport or national ID), I had no reason to use it, most of the people here wouldn't know how to spell it and I didn't want to complicate my life. Thanks anyway for the reply.

That's not what you wrote in your original post - you wrote there that you have used your married name on a bank account in the USA.

The question does qualify how, when, or where one has used another name, it simply asks for any other names used.

So which post is correct - you have used your married name or you have not?

But you're right, I have used it and I will put it on the form. That's all I wanted , a simple answer.
