Hey Everyone,

please read the following info taken from the embassy in Caracas, Venezuela:

Medical Examination Results: The U.S. Public Health Service requires all immigrant visa applicants, including minor children, to have a medical examination at a designated facility. The Immigrant Visa Unit cannot accept medical examinations completed by a physician in the United States or by the applicant’s own physician. The applicant must schedule an appointment with one of our panel physicians. Please download the four (4) Medical Application forms DS-2053, DS-3024, DS-3025, DS-3026 and take them to your medical appointment along with 3 color photos, your passport, and your vaccine records. The results of the medical exam will be valid only for 6 months from the date of the exam.

So my question is where do you find them??

If one looks on google it shows the instructions coming from the CDC's website, but the forms are not there... it directs you to look at the DOS's website which it does not have those forms, or at least I cannot find them...

Thanks a lot... it would make much more sense if each embassy had them... or unless I am not to internet savvy... to find them in the Embassy's website for CCS at least... thanks again...
