Appointment of Agent form?

I was given advise to get one of these forms from the uscis website and have her sign it and have it notarized before leaving the Phils. I can't find any such form on their website though and the only similar form I found online was for tax purposes. I was told it was similar to a power of attorney but exclusive to immigration. I assumed it was so that I could submit forms on her behalf without spending weeks sending documents back and forth via mail or DHL. Can anyone tell me if they've used or even heard of this?? Thanks!!

its a DS3032 "Choice of Agent" form. yes she have to sign it but no need to be notarized.

as i remember we just waited for them to send us chioce of agent form...when my hubby received it he scanned it and sent it to my email and i signed it and sent it to nvc...maybe from other people here they did something faster.

actually that choice of agent will be sent to you if your petition is already approve...so just know first the documents for filing the petition...its either you will go for k3 or cr1.

And also, print a copy of DS230 and asked her to sign the part 1 only and don't date until it is need to be sent to NVC. Good luck.

The forms that start with DS can be dowloaded from the Department of State website :-)

02-23-2018: Sent N-400 Application online

02-23-2018: Date on NOA, retrieved from online account

02-23-2018: Date on Biometrics Appointment Letter (Biometrics Appointment at Jacksonville ASC on March 13, 10:00 a.m.)

03-08-2018: Biometrics complete

04-05-2018: Case status updated - Interview Scheduled on May 10, 2018, 10:15 a.m. :D

05-10-2018: Citizenship Interview - Passed English and Civics Tests, Recommended for Approval! :D 

06-19-2018: Received email and text notification: Naturalization Ceremony Scheduled; waited for letter to be uploaded on online account - it has been set on Wednesday, July 25, 3:00 p.m.

07-25-2018: I am now a U.S. Citizen!

The DS3032 choice of agent form can be sent back to NVC by email. You can use the template here on VJ Here You do not need to send it by Snail mail. Also have a read of the NVC guide it will explain exactly what forms you are going to need and what documents you will need to get to go along with the forms. You can find the guide NVC Guide
