transcribing I-693

hi everyone!

i have an appointment on wednesday to a civil surgeon here in Florida,.i talked to his secretary about it and she said i need to have physical exam, well i told her that i dont need to undergo physical examination because i already have that in the Phils.,and it is according to the instructions of I-693 in the uscis website for a K-1 Visa holder . I think she's not aware of that but i said i'll bring a copy of that to show u and the doctor,..and she said ok.

if it will not work out there i think i will have to have my I-693 transcribed by a civil surgeon in Texas. to those who did this,..what did u send?original or photocopy of vaccine record from St.luke's?cd from St. Luke's?how long does it take?and it says applicant should sign the form in front of the civil surgeon.?

call the Dr's office first and they will tell you all what you need to submit

We got the same thing from the surgeon in the Minneapolis area. When i showed the instructions and that the date of the physical in Phil was less than a year he said okay, had me pay $30 and we got the I-693 in a sealed envelop along with a copy.
