I know that there is a lot of info on that here, but one post contradicts the other or the info is out of date - that's my excuse for asking and my question is:

Which form for affidavid of support must the petitioner fill in and if we need a joint sponsor than which form is to be filled by joint sponsor?

TIA

Oh and the other question is if it needs to be notarized.
