Hi eve
just bring the ff:
1. passport
2. payment more or less 10,500php
3. the letter you received from Manila USem.
4. as a back up, bring your NOA1 NOA2 have it photocopied.
5. bring (3) pics 2x2 white background(us passport photo)
6. also bring your vaccination records if you have any and have it photocopied, if i remember it right 2 copies of them.
Remember when was the first day of your last menstruation they always ask for it during the exams.
i also have a history of thyroid problem, they checked it and asked if i had my medications when and how long
i would suggest you go there at noontime after you had your lunch para di na masyadong maraming tao, thats what i did and it worked the process was easy just expect for lots of waiting lol.
Goodluck! you will make it!

Thank you for posting this question, and good luck. I think many of us wonder about the exam. I don't mean to hijack your thread, but may I ask? Is a copy of the email from US Embassy in Manila stating "you can schedule an appointment for your interview" sufficent, cause this is what we received, as we have not set an appointment date as of yet. I heard your also suppose to bring a black ball point pen with you. If you have had no immunizations in the past or long ago, do they charge extra for the needed immunizations you would get there, or are they included in the $213.35?

Actually, that letter from the USEM is what they are looking for for you to get in in SLEC. You can buy black pen outside SLEC if you want and you can also bring your own. You just have to have black pen as you'll fill out and sign lots of papers every now and then. NO, they won't charge you anything if you don't have your immunization records with you and yes, all the vaccines that they will give you is already included in the fee.
Good Luck to all who will have their medical

So to make this clear in my mind, will a copy of the email from the US Embassy indicating we can schedule our interview date, work? We don't have an actual letter, only the email.

Hi, IF you did not receive any letter from USEm Manila, i believe that a printed copy of that email you received will suffice, just bring noa1 and noa2 as back up. if the SLEC tell you that they will gonna give you shots of vaccines, that's fine kasama yan sa binayaran natin. At gamitin mo lang yong regular black pen ok na yon NO need for 0.5 or 1.0 sign pen hehehe...
Just a few reminders on your interview:
1. make sure that you pay your visa application fee at designated bank.
Bring the receipt and they will issue you a blank ds 157.
(i did mine at BDO) this is to avoid delay during the day of your interview.
2. do not forget your APPOINTMENT LETTER that you received from USEm Manila.
the guards at USEm will check that before they let you get inside.
OR the confirmation email that you received about your appointment interview date, have it PRINTED.
3. Be confident, pray and trust that everything will be alright. Don't forget to smile at the consul.

(1) notification letter from US embassy manila was received on 20 may. letter indicated time and date of appointment for medical exam and interview.
(2) 26 may 09 -- arrived at st. luke's extension 430am and i was number 20 in the queue. guard opened the clinic at 6am and started distributing the forms to be filled-up (form for x-ray, blood test, P.E., vaccine). the reception opens at 7am. they will call the first 10 pax and take ur digital photo then proceed to the 5/f for payment. a flow chart is provided so don't worry about the sequence of the exam.
(3) for the P.E. (which i was most anxious about), the doc would ask you several questions (see form on medical) regarding your medical history. after that she will ask you to disrobe and will conduct breast and external genital.
(4) i didn't get my results until 28 may 09. others were able to get theirs the following day. dunno how st. luke's does the scheduling for the release of result. thought this was the easy part but had to wait again for 3.5 hours before they release the result.
(5) don't forget to bring some snacks (na pwedeng kaining patago dahil bawal kumain dun) and water maybe a liter if your a drinker.
good luck to those going for their medical exam.

Are you sure that three passport photos are required? Everything I read is one passport photo. Can anyone clarify that? My fiancee will have her medical this Thursday (instead of Tuesday). I don't want any difficulties for her. Any help you can provide will be appreciated.
Best wishes,

am 100 percent sure 3 passport photos are required -- 1 to be submitted on the first day & 2 copies to be submitted on the second day of the medical exam when st. luke's also collects the passport.
good luck and relax.

2x2 inches photo with white background is the standard passport photo for US. hence, it is a passport photo based on US spec (http://travel.state.gov/pdf/Photo%20Guide%2010-01-04.pdf), w/c will be attached to the form you filled up at st. luke's on your first day of medical exam. the other 2 copies of the photos will be enclosed in your passport to be forwarded to the US embassy. you will need to attach 2 passport photos to your form DS-156 (2 copies) later on for your interview at the US embassy.
