My Vaccination record copy from my medical exam says "For use is Canada only" on the top of the page. If I need to transfer my vaccination record so I can adjust my status, Can I fax/send this copy to the texas office and have them transfer my info?

Yay!!!!!!! I am doing my AoS this weekend and was wondering this myself. From what I am gathering, there are 2 possibilities.

A) You can just send in that form that we got in Canada and hope you don't get an RFE (I have been reading some do, and some don't, sounds like it depends on your vaccination record and if you were all up-to-date).

B) Download an I-693 medical form, locate a Civil Surgeon in your area, and take the form in and he will do the rest. Not sure exactly what they do//fee involved etc. I am going to call mine Monday.

BUT, here is a great FAQ that states you more than likely don't have to submit anything but that letter you got in Canada from your medical. Thats what I may do.

January 23rd, 2009 - Montreal finally gets it. "Overnight" my a**!! Money refunded to me from Canada Post...ASAH!

March 27th, 2009 - Notified by phone call of interview date

March 31, 2009 - Interview Letter received in mail (P4)

April 6th, 2009 - Medical

April 17th, 2009 - Interview!!!

April 27th 2009 MOVED POE - Buffalo Peace Bridge

May 1st 2009 - Married!!

May 18th - SSN arrived in mail

AoS:

May 14th, 2009 - Filed for AoS - Come on NOOOO RFE'S!!!

May 22nd, 2009 - NOA1 for AoS, AP, EAD - California Service Center. ASAH

May 30th, 2009 - Biometrics appointment letter received for I485, I765 (where is the I131????)....date June 12th. 2009

June 2nd, 2009 - Biometrics completed

June 15th, 2009 - EAD Card production ordered!!

June 20th, 2009 - AP, EAD arrives in mail! (no email from CRIS)

July 2nd, 2009 - AOS Transferred to California

August 7th, 2009 - Green Card production ordered. WoohoO!

I also submitted just the vaccination worksheet. I had all of the vaccinations I required or had had the illnesses themselves and had natural immunity. I didn't receive an RFE and was approved as is. If your vaccinations are complete, give that a try - include a copy of the worksheet with the AOS application. If you do get an RFE then you would be able to get a Civil Surgeon to complete the 693 Vaccination Record for you and send that off. I believe you should be able to use the office of the doctor in Texas who does them by mail.
