hello just want to ask again...in the form ds230 it ask about aliases or other name use, if married woman give the maiden name...ex my name is Joan Lim when i was single then now i am married My name is Joan Lim Smith....do i answer Joan Lim on qustion 2 or i will just put Lim on it...pls help thanks in advance

I just put my last married name,( the ex) followed by my maiden name....

I just put my last married name,( the ex) followed by my maiden name....

MAIDEN name means the original name which is in ur BC

example

a lady names jolly smith ..

after marriage she become Jolly Smith stephan

so the Miaden name is Jolly smith

but i see guys here say they wrote the married name and they got no problem so its up to u to put what you like but i just wanted to answer your Question

Just to clarify this is how I filled it in: I specifically put (first mariage) after that name and the same for my maiden name.

JANE DOE (FIRST MARRIAGE) JANE SMITH (MAIDEN NAME)
