The form has your personal information on it so I would have to make one rather then use a copy of someone else's. Here is one that a fellow Canuk uploaded for us to view.

Lil Mermaid - I did highlight my case number on a cover letter when I sent back my Packet 3 forms to Montreal, so I think you're right about waiting until you get your actual packet before sending anything in. Let me know if I can help you in any way.

Our Timeline

March 21, 2009 - I-129F sent to VSC

November 6, 2009 - Interview in Montreal - Approved!

February 15, 2010 - POE Thousand Islands

March 27, 2010 - Wedding!

AOS

April 17, 2010 - I-485, I-765, I-131 sent to Chicago Lockbox

April 26, 2010 - Received NOAs for I-485, I-765, I-131

May 13, 2010 - Transferred to the CSC

May 19, 2010 - Biometrics appointment

June 17th, 18th, 22nd, 23rd & 30th - touches on I-485

June 25th, 2010 - E-mail: EAD approved

June 30th, 2010 - 2nd E-mail for EAD

July 1st, 2010 - Email: RFE being sent for AOS

July 2nd, 2010 - AP received in the mail

July 6th, 2010 - EAD received in the mail

July 8th, 2010 - Received RFE in the mail. They have no record of my medical!

July 12th, 2010 - RFE (new medical) sent back via Express mail

July 21st, 2010 - Welcome Letter (dated July 15th) received by mail and e-mail received stating GC approved

July 23rd, 2010 - GC received in the mail

Timeline

Just wanted to let everyone know, Monday Janus got his visa. He went to the interview last Thursday, which consisted of only 2 minutes of questions, followed by an HOUR of them asking for forms, him finding the forms in the binder, and then handing the forms to them. Most of them time he was there he was either waiting, or handing them a form.

And then 3 days later they emaield him and said to pick up his passport with the sticker in it.

So that chapter is almost over (he still has to fly over in December, have the 'thing' at the airport, and then we get married New Years Eve). And then of course, we file for adjustment.

But all in all, an easy interview experience for him. Kind of annoying with the interview waiting time, ridiculously long and stressful with the application time, but we are very grateful that part is over.

Also, the day after his interview, his boss had a meeting with him to tell him: his company has agreed he can transfer to their office in San Francisco. So now I have to pack up my life in NYC and looks like we are moving to the West Coast!

So much is happening so very fast, after soooooooooooooooooooooooooooooo many months of waiting!!!! Ain't that life?!?!!

This is wonderful news!!! Wonderful indeed. By the way I am coming to NYC thursday and Friday. I might stay the weekend. I was thinking I might even try to crash Roman's wedding on Saturday. Roman is on the feb thread. If I do end up staying we could go grab a beer or dinner if you have the time or inclination.

But great news!

Well, just got back from a day in Montreal to have my medical. The Medisys office in Montreal is quite professional and efficient! The doctor was really friendly. We spoke about Disney World more than anything else. I don't have the official results for another 3 days, but he said he is going to tell the embassy that I am a "very smiley, healthy 26-year old." lol.

Brian flies in on Friday night for my sister's wedding that is on Saturday! We really have to make the most of this trip because I'm not moving until February. (I am determined to save up a healthy amount before the big move and have my car all paid off!)

Hope you guys are all doing well. Richard - I'm glad to hear that they found your file and scheduled you an interview!!!

Disney world? Are you by chance moving to Florida as well?

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

Thanks, she was really, really old and not really having any quality to her life and then she got sick and it was just a mess. NO MORE PETS!!!
