g 325a employment Q

Hello,

I'm filling out the g 325a form & I came to the employment question. I am self-employed. In the last 5 years I've worked at 2 different locations, but still remained self-employed. Now, I put my name as employer(correct??) but what address do I put? The location where I worked or my own? I did/don't work from home.

any help would be very appreciated!!

I would put whatever address you used on you tax return for your business.

Employer: "Self" or "Self Employed"

Address: If it is the SAME business, same business name, same type of business, nothing chnaged but the physical location, then put the current official (legal) address for the business and let it go at that.
