Common, no. But they do happen.

Actually denials are very common but most denials are because of lack of some document. Those are usually overcome by submitting the missing documentation. Prepared couples with bona fide relationships are rarely denied.

Very true, with one additional consideration to mention: At the U.S. consulate in Guayaquil, Ecuador, a couple can be as prepared and "bona fide" as possible, but if the U.S. partner does not attend the foreign fiance(e)'s first visa interview, the visa will almost certainly be refused, or delayed interminably by never-ending requests for "more evidence." Furthermore, the consulate will never tell anyone this. Accordingly, research the happenings at your eventual consulate -- read the appropriate reviews here, and send a PM to other members as necessary in order to learn the true inside story.

What we think is quite irrelevant as we are not the ones interviewing your fiance, nor are we the ones making the decision to issue the visa. Take the good advice given to you in the thread, make sure you are prepared with all your relationship supporting evidence and any other requirement that your consulate has.
