I checked with my Mom and as a child I did get the MMR and chickenpox vaccine. From what I have read and understand online those vaccinations are good for life, however I have no record or proof that I had them. I am currently trying to see if my family doctor has a record of this, but I doubt it. I would have got them in Quebec 30+ years ago and tracking that down will be a headache dealing with Quebec, but I think this is where I would start: http://www.msss.gouv.qc.ca/en/index.php (From experience in trying to obtain any of my records from PQ as an English speaking person from Ontario it would probably be much easier to just get the vaccinations again, if that is possible.)

I will very likely need a tetanus and diptheria booster. They can give me the chest x-ray for TB and the blood work for STI's at the Visa medical.

Are any other vaccinations required for entry into the US with a K1 Visa?

I wasn't sure either if I had chickenpox when I was a kid .. so they went ahead and gave me the vaccine anyway. If they are unsure, they will give you the needed vaccines, you do not need to worry with that :)

Also there is a list of required vaccines included in the packet 3, but only the panel physician can determine which ones you are eligible to receive. A lot of these vaccines depends on your age and medical history and they will tell you at the medical exam which ones you need :)

That is probably the best description I've seen around here about VJ. Being here (VJ) helps keep you sane is almost an understatement! So much support and people to talk you down out of a rant about the process and waiting... wow. I had no idea how amazing this place was until I started to really read the threads! :thumbs:

I went in to see my family physician today and got shot with the MMR vaccine, Diptheria/Pertussis/Tetanus vaccine, and had blood work done to see if I have chickenpox antibodies. Results in 2 weeks.

I believe this will be a good head start on the Visa medical, and at this point is all I can do in advance to get some stuff off the checklist?

I am not sure what a "long form" birth certificate is? Is this something I will need for this process? The current birth certificate I have is from Quebec, and is only a small credit card size paper document, with my Name, Sex, DOB, POB, a Registration #, and a Document #. All the print on the reverse side is in French. I Googled long form BC's and I believe I have the condensed version...this is not acceptable?

I'm not sure about the forms in Quebec, so that might be enough. But I know that they need a birth certificate with your parents names on it. SO perhaps there is one you can get in Quebec ? You might have to do some research on that.

Your long form birth certificate contains the names of both of your parents. Mine is also from Quebec - it is not the wallet card so send away now for your long form certificate. It is required. If it is in French, I believe you will also require an English translation. The Quebec Government will only issue the document in the language in which it was originally recorded.
