I am filling out the G-325A for myself and still have my married name (I was previously married to my kids dad). So should I use my current married name or put my maiden name?

Thanks

Use whichever name you use now. If you still use your married name, put that one.

I used my married name (my first husband) until I married my current husband.

Thanks...I thought they would want the current name, but to me, family name implies maiden. THanks again.

Isn't there a question about other names used? That is where you would put your maiden name. I know they ask for it on one of the forms, not sure if it is the G325A. But yes you fill out all the forms with your current last name.

Good Luck mrsladyd!

Where it asks for "name", give your current legal name as in your passport. Where it asks for "other names used" list your maiden name.
