name filling

my lawyer sent me the consular packet today. in the form ds-156 he has put 'john' in the column for 'surname as in passport' i dont have a surname in the passport. john is my fathers name we use it as our family name and that is what we filled in the visa forms that got approved( we had also sent copies of my passport showing this to them). so as such my passport shows i dont have a surname. can i leave that column empty in form 156. i filled john as family name in form 230 as they didnt ask as in passport ask have an interview date. i hope everything goes smoothly

any help???? anyone who did the same.

also if the lawyer has filled the form 156 doesnt he need to sign it.... he hasnt signed it.
