Medical forms, please!

Hi! I'm such and idiot!! I recycled the Medical forms the embassy sent to me , don't ask me why. After I threw them away the doctor told me to take the forms with. Anyway: I tried to download them from the Internet, but I have 2 tiny questions:

1- There are 4 different forms, right?

2- The forms I found say they expire in 2007. Can I use those? I couldn't find a more recent version.

Ignore the OMB date at the top of the form, that date is not the form's expiration date, it is a date that DOS has to complete a survey conducted by Office of Management and Budget, for the paper work reduction act.
