CONFUSED..I paid the $131 visa application fee

I paid the visa application fee yesterday at BDO Ayala..

I have a question.. when I paid out the fee, they gave me a SUPPLEMENTAL NON-IMMIGRANT VISA APPLICATION which is blank. I think its the (DS-156K form-not sure..) Do I have to fill it out? Cause me and my fiance hired an Immigration lawyer and was responsible for handing me out all of the DS forms that are needed. I already filled that form which was given out by our lawyer.

So, do I still need to fill up that form that the bank gave me with the confirmation that they received my payment which was posted at the back of the form?

Hope you all my fellow VJ'ers could help me figuring this out.

It should be a DS 157. Fill it out and make 1 copy. You need 2.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

You have to fill out that form (DS-157) given by the bank because if you noticed there's a bank validation at the back of the form. What i did when i paid mine i filled out that form online and just presented to the bank, i didn't get the form from them.

here are the instrutions for the forms needed at embassy:

None of the official K1/K2 visa application forms will not be mailed to the beneficiary, as of March 2008, but may be downloaded from the Department of State (DOS) website [CLICK HERE] which is indicated in the USE Confirmation Letter mailed to the beneficary once the hard copy case files has been received by the USE from the NVC. As of 01 November 2006, the DS-156 (Nonimmigrant Visa Application) MUST be completed online via the Electronic Visa Application Form (EVAF) http://evisaforms.state.gov, which is sited on the DOS website. Completion of the DS-156K (Nonimmigrant Fiancé(e) Visa Application), the DS-157 (Supplemental Nonimmigrant Visa Application) and the I-134 (Affidavit of Support) can be done via computer (recommended), type-writer (just as good), or handwritten (last resort). I would make at least 2 copies of everything, we even made spare copies to keep for our records. Good Luck

Not sure if this part helps, but Radz had the form filled out online via computer when she went to the bank in Davao, the bank would not accept that form and made her fill out a new one, That probable can change from bank to bank, but that is what happend with us. Radz said the bank told her she had the old form, After that I checked then online form again and it was the one we had used so not sure what happened there, we were just happy to have validated form. LOLs

The above comments are correct.

The bank gave me form DS 157 with a USEmbassy stamp and a bank validation at the back.

Remember, u need to copies of this, so, u can photocopy it first. I am glad we did not get lawyer for our visa process. VJ is helping a lot better than a lawyer..lolz
