vaccination documetation worksheet (DS-3025)

I would like to ask about the Vaccination documentation worksheet that was given from St. Luke's. I was given 3 shots namely: Anti- Tdap, HPV, MMR(measles-mumps-rubella). Only MMR has a check under completed series and the rest are insufficient time interval (not completed).

I would like to know when will I go to the doctor to have my next shot?

Do i have to get that shot before I will go to a Civil Surgeon to have my DS-3025 (vaccination documentation worksheet) change to (I-693A) Supplemental Vaccination Record to be submitted along with my other requirements to USCIS for my AOS? or

Is it possible to go directly to the Civil Surgeon and present my DS-3025 and ask to change it to (I-693A) Supplemental Vaccination Record?

A civil surgeon will review what immunizations you have had and then determine if any additional immunizations are required. My recommendation would be to contact the office of Dr. L. E. Arnold in Austin, TX. The fee is $20 and you can reside anywhere in the US and use their services. If additional immunizations are required, they will advise you which ones to get and then you submit the documentation to the Doctor's office and it will be transcribed on to the I-693. If you go to a local Civil Surgeon they may try to pressure you in to getting another physical (not required) and most likely will charge you a lot more than $20 to transcribe your information on to the I-693.

Call Dr Arnold when you get here. I don't know if she is still doing the I-693 transfers.

I think we did show the CS the 3025 document, he requested it. Hang on to the xray too, the CS wanted to do a new one until I whipped out the one Claudeth got from St. Lukes. That saved us seventy bucks. Of course Claudeth thought the seventy was hers so I still had to pay
