Form DS-0158 Expired! New form ?

Hi all,

Just a quick question as the topic states. The DS-0158 form available on DOS Travel

website carries with it an expiry date of 04/30/2006 and has since expired in April

this year. No updates or news or even links about the newer version of this form is

available anywhere on their website, so far as I have looked into it.

Any ideas from anyone who have noticed the same ? What to do if one needs to

apply using this form ? Thanks.

If you need to use the form, use it. Expired or not it's the most recent there is.
