I have been looking at the guides and wikis for this and am still at sea.

LLings' Shortcut and James's Shortcut reference a Barcode Cover Sheet, and a Personal cover sheet, but I can't find templates for these. We never actually received this package of documents from the NVC, 'though we've paid the AOS and IV fee bills a few months ago, so I want to generate these myself. I understand that I need to create a barcode, for which there are ample instructions, but without the template form, I can do nothing. Is it still necessary?

The actual I864 is available online no problem, and we will fill this out and we have (or are in the process of getting) the tax transcripts etc. Letter of Employment we are also okay with.

A personal cover sheet isn't a requirement to send it, but I made one anyways. Basically, it just included everything that was being sent in, listed in the order they were put in the packet.

James's Shortcuts are mainly for creating bar-coded cover sheet for NVC documents (I-864, DS-3032 etc) back in the days when online payment system was not available. Main objective of James's shortcut was to eliminate waiting time in post when NVC sends back barcoded coversheet for I-864 upon $70 payment of Affidavit of Support Fee.

If you paid AOS fee bill and IV fee bill via online payment system then you can print barcoded coversheet directly from the website you used for payment. NVC will not send you any barcoded coversheet. For some users when site did not work then they were able to generate barcoded coversheet from James's shortcuts and it was accepted. James's shortcut and other guides often mix up with cover letter and barcoded cover sheet.

Your timeline has large gap in dates and there is two paths for CR1 and K3 so it is little hard to judge question you asked. Please update your timeline.

02/01/2008 : Mailed I-130 to Chicago Lock Box via Priority Mail

02/04/2008 : I-130 reached Chicago 2:18 AM

02/19/2008 : NOA1

02/20/2008 : $355 check cashed

02/25/2008 : Case available to view online

08/26/2008 : Email: Case will transfer to CSC

08/27/2008 : Notice Sent: Case will transfer to CSC

09/04/2008 : Email: Case arrived and pending at CSC

09/05/2008 : Touch

09/08/2008 : Touch

09/23/2008 : Approved !!! (217 days | 31 weeks) from NOA 1 (235 days | 33 weeks) from file

10/10/2008 : I-864 Fee bill shows status "PAID"

10/14/2008 : I-864EZ Package sent via priority mail

10/16/2008 : I-864EZ Package reached NVC

10/16/2008 : E-mail for Choice of Agent accepted. (12 days)

10/16/2008 : IV Fee bill ($400) submitted online

10/17/2008 : IV Fee bill accepted

11/13/2008 : DS-230 Package mailed to NVC (Had to wait for PCC)

11/18/2008 : DS-230 Package reached NVC

11/21/2008 : Case Complete (53 days | 7 Weeks) at NVC

01/27/2009 : Interview (361 days | 51 weeks) from file

02/09/2009 : POE (Newark)

02/21/2009 : SSN Received

02/25/2009 : Welcome Letter Received
