I am filling out the G-325A for my Canadian spouse, who has lived here in the U.S. since 2003.

One of the questions on her G-325A is "Applicant's last address outside the United States of more than one year."

Am I to fill in her last known address in Canada, where she resided for about 14 years until she moved to the U.S., or do I leave that blank, since she has lived in the U.S. for over 5 years.

Thank You.

That is a good question. How did your spouse live in the US for 5 years? What kind of visa were they on? or were they even legal? If they were not here legally, you could have problems.

It is asking for the last address outside the country - no qualification other than "for more than one year" - so, I'd list the address you mentioned.

Also, do not leave fields blank. Either put "None" where it truly means that there is nothing. If the question is really not applicable to you, put "N/A". There have been instances where people have received RFE due to blanks.

Do NOT put N/A or None on signature fields which you don't need to sign. Leave those blanks (ex: the field where it asks for signature of person who filled on behalf of petitioner - you would leave this blank if you filled the form yourself)

there are sample forms here if you want to look at them as reference :)
