Question about Vaccination Supplement Sheet for AOS

Hi!

I am about to send my application for AOS + EAD + AP and I have the following question about the Vaccination Supplement sheet that is required.

When I had my medical examination for my K1 visa in Montreal they gave me a sheet called "Vaccination Documentation Worksheet"(DS-3025 apparently) (it says on this sheet "for use with DS-2053").

Is it the sheet that I need to submit along with my AOS application?

What about the medical documents that are in that big brown enveloppe that I still have and that was never opened?

The brown envelope from your medical will be handed over at your POE (DO NOT OPEN IT) and they keep it with your file.

For your AOS process, you need to bring your vaccination worksheet you got at your medical to a civil surgeon and have him/her transfer the info to a vaccination supplement sheet (I-693A). However, if it's been over a year since your medical, you will need to get a complete medical done by a civil surgeon in the US and the doc will fill out form I-693 and you will include that with your AOS paperwork.

I just sent in the page that has the doctor's check marks next to all the vaccination names. The one that you got with your medical exam in Canada.

Many recommend getting a US doctor to basically copy the medical results down on a new form. But to me it's going to read the same either way.

They accepted mine when I sent it in.

OK thank you very much for your advice! I think I'll do the same as you! If ever I get a RFE I will then contact a civil surgeon to fill that damn i-693.

Yeah, by the time I'd bought the $1010 money order for the AOS/EAD/AP deal and having paid fees for all the stuff along the K-1 journey, the last thing I wanted to do was pay a doctor money to "copy and paste" my paperwork that was prepared by a doctor certified for US immigration.

you say your k1 , are you already in the states ? cose to my knowledge you was supose to have hand over the brown seald envelope when you crossed the border ( when you activated the K1 ) I hope thats what you did ,

and hope you didnt keep the brown envelope when you crossed the border
