In my case, I lined up at the DFA at 2 am only to be informed that my place of birth does not indicate the city where I was born, even though the municipality is indicated in the upper left hand corner. i had to have a supplemental Place of birth affidavit notarized, then i had to submit it to the city hall. i had to wait several weeks for that. after picking it up, i had to submit it to the NSO. again, a wait of several weeks for NSO to produce the annotated birth certificate. it cost me around P750 and a huge headache.

1K and a huge headache is a bad combination, sorry to hear that. I don't like the "several weeks of waiting" and the "huge headache" and the "1k" but these are really a part of this process hahaha ayyyy I have to prepare my self next week for this haha!

Thanks for sharing...

You really have to prepare yourself for the possible headache that you'll be encountering regarding correcting errors on your BC,i had mine corrected thru RA9048,a one letter error on my last name and i've to put Ma. on my first name coz i've been using that eversince.it took 4mos before i got my annotated BC from NSO and it cost me less than 5K pesos.

As for the Baptismal Cert,i had mine annotated at the Arzobispado de Manila located at the back of Manila Cathedral. My name on my baptismal certificate is totally different from my BC and told me they cannot just follow the name on my BC since it was the name given to me by that time,so they just handed me a letter which i turned it over to our parish church and they just put a remarks(annotations) instead.

That was a real BIG headache on my part but i'm so glad it's all over now,i have here all my docs annonated :)

Hi thanks for this info.

I have one letter error on my last name (just that)... Really I have to wait 4 months? We are expecting NOA2 this month or next month (God willing)... If I start fixing it now and it will not be annotated in time for interview, what will be the possible problems it might cause?

(BTW: My passport has my last name corrected way back...)

Thanks for the inputs...

In Manila registry office, everything's slow so that's the reason i guess why it took mine 4mos,i just hope you're not registered here, some municipalities they said are fast and easy.i'm suggesting you start working on it now.As for my Ma.i had it published for 2 consecutive weeks at the local circulation.in your case, there's no need for it to be published since it's just a typo, usually the transit from local civil registry to NSO and from there to LCR are the ones that takes time.

i hope yours will be fast

Oh,you better get it done fast.Better bring with you your school records, parents' marriage cert.,your father's last name since it's the one that has a typo on it and other records that'll show the last name that you've been using eversince. I'm not sure what will happen if you dont have it annotated yet at the time of your interview.

you're welcome :) and good luck

:crying: Di na ako mapakali regarding my daughter's birth certificate upon learning it takes 4 months to wait. What do you think will be my move, my name on her birth certificate appears to be MA. instead of MARIA, should that be changed or will an Affidavit of discrepancy be acceptable? This was caused by a manual error, not knowing they did until the original birth certificate was released, grrr.

Need some opinion mga sis! Thank you so much!

I am hoping hinde nila patagalin yung sakin, I only need one letter on my last name to be corrected... Sa Manila ka rin ba naka register? Ang tagal pala before makakuha ng barcode copy ayyyyy!

Thanks for sharing I know you're still celebrating dahil sa Medical Results mo.

Documentation consistency is a huge problem there. My asawa's school docs have her first name spelled much different on her elementary, high school and college diplomas/certificates. None matched her BC. Part of the problem is the extreme reluctance of uneducated parents to correct the spelling early on, especially when an educated school administrator determines the child's "correct" spelling upon enrollment without asking the parent. So, the problem is ignored, often for years. Even my asawa when she was older didn't feel it was her place to correct the problem. The end result is your supporting documents often raise more questions than establish continuity. Thankfully, her NBI was correct and all of the important legal docs did match.

This happen to me too. We learned that my name in my BC was wrong including my family name when I was 24 yrs old. It cost me around 10 thou pesos to correct it and at that time it has to go to court for correction it took 3 yrs and so much hassle.

Cloe, If you have it corrected get a new copy from NSO because what happen to mine was that the judge had ordered the change in my BC but the records at NSo still the same for 3 yrs and I was just glad i had a classmate in college who was in manila at that time and he helped me to follow it up. God bless you.

Hi Riza, yup,it was like that before, you have to go through the court in order for your name to be corrected and i'm glad we already have this RA9048,although it'll still take some time,it's still good coz we dont need to hire for a lawyer and will wait for years before it'll get corrected.as for checking if it's recorded at NSO,she need not to worry about that because during the process, the LCR will forward the request to NSO and the NSO will send it back to LCR and as soon as her petition is granted,LCR will give her Decision and Finality(a CTC BC with complete annotations is already included) and will ask her to bring it to NSO East Ave.Area B,the people there will check all her docs and if everything's ok,she'll be asked to pay at the cashier to apply for her Annotated copy,it'll be released after 15 working days  .She just have to remember to have it photocopied so if ever she'll apply for another copy, she could just attach it (BC xerox copy) with her application form, that way it'll be easy for NSO people to track her BC

I guess in your case, since you've followed what's on your passport,an Affidavit of Discrepancies will do.just my thought, maybe other members has other infos to share with you.
