G325A Form Question

I am just taking a look at the G325A form, I have a question. The 5th line down it asks Form Husbands or wives name. In the next box it has first name, but where would I put his last name? Stupid question but I don't want to assume with the VSC!!!

Right underneath where it asks is the space to put the 'family name'. Here's an example form to help you out.

I just found it thankyou so much. Another question. it indicates to put the date of marriage and the place and country, I can't fit it all in on one line, any suggestions. I see you will be moving to Mass, I will be too, I just love it there. It reminds me alot of home here, I am only 50 mins away by plane so very lucky.

If you can't fit it in by typing it, then after you have printed it just write it in.
