For new applicant of Philippine passport, if the minor is 7 yrs old and below, mother will fill out the application form and thumb mark.

-4 passport size pics, with white background and wear something with collar

I think the 4 pcs pics only and the application form that you will fill for her. My sister applied Philippines passport for her 3 months old baby before and that's only she brought.

Hi Gurl!!!

if you will be the one to process personally at DFA it costs only P750. But if you go to a travel agency it costs 1200-1500 depends on the travel agency. It is less pagod pag sa agency ka compared to ikaw ang magprocess personally.

For the requirements shape mentioned it already.

During your appearance in DFA you should bring ur passport. For babies 1-7 years old you dont need to bring the baby during the appearance. The most important is your passport.

Here are the requirements:

http://www.dfa.gov.ph/consular/passport-eng.htm

Please note that the background should actually be Royal Blue rather than white background. Also is it supposed to be 4.5 cm x 3.5 cm (note this differs from the size and color the US Embassy requires for the visa photos). Make sure the child is looking straight on at the camera rather than their head tilted up or down, etc.

My fiancee had to have the pictures retaken of her child at DFA since her head was tilted down after having had them redone since they were put on white background at a photoshop instead of royal blue. This is new as the old passports used the white background.

Only ID needed for her child was a birth certificate. My fiancee brought her own NBI, birth certificate and passport along.

Be prepared to apply for the passport, then given a date to come back to pay and finish application a week later. Then about a week after that to have the passport ready for pick up.

Hope this is helpful,

Jason

Hi Jonah,

This are what u need for Sophie:

Passport mo photocopy...bring mo din original passport mo

Passport copy ng daddy nya kc gamit nya apelyido ng Dad nya

4pcs.passport pics royal blue background

baptismal certificate ni Sophie

NSO birthcertficate ni Sophie

ID's mo

500.00php kapag 2weeks

Tinanong ko yan dun sa DFA nung nagpunta ako nung nag-apply ako ng passport ko.Tinanong ko just in case na hindi ma-acquired ng anak ko US citizenship nya,Philippine passport gamitin nya kc naka-include din nman name nya sa K1 so K2 derivative sya.But Hopefully sana ma-acquired nya US citizenship nya para US passport gamitin nya para di na sya magmedical at mag-CFO.

Good Luck!!!

I applied for my 16 months old son then, last March and I only had the form, 4 passport size photos against a royal blue background, and his Birth Certificate in SECPA from NSO.

Note:

If B.C. is not yet available at NSO, you should ask from your Civil Registrar's Office if it was forwarded to NSO, if it was then you need to wait for a few days or months (depends on what place you're from coz BC is still being processed at Manila NSO Main Office), it it wasn't forwarded yet to NSO, ask for a copy of Form 1A from Civil Registrar and bring it to NSO and have it authenticated.
