So we made the very stupid mistake of not making a copy of my fiancee's passport before her medical. Now SLMEC has it and we are trying to fill up the DS-156 and its asking for the date issued and date exp. She is pretty sure of the date, but not 100%. Will we see the passport again at the embassy to check? Maybe we can call DFA and give her passport# and get the date issued? this sucks so bad...I think I'll be sick now... :bonk:

Hi Polar,

About the passport number , it's in her vaccination record copy that SLEC gave your fiancee. Have her look at it, it's there. ;)

Shape

Ah, yes she does have the passport #, but she is not completely sure of the date issued, and this is asked on the DS-156. :(

hi in PI, the date issued and the date expire is the same month and day except the year== that will be 5 years from the date issued....

ex:

date issued: NOv 27, 2007

expiration dat: NOv 27, 2012
