WE are at the NVC on CR1. My wife has two NSO birth certificates, two NSO marriage certificates,postal I.D. and an old picture I.D. from college. She will be going to get a passport next week. Will there be any other documents needed before medical, CFO? We are trying to get all that is required so not to be delayed. T H A N K S

- philippine passport for your wife

- 4 pcs ( white background) 2x2 US visa photos

- NSO certified marriage contract

- NSO certified birth certificate

- NBI ( uptodate)

- police clearance (uptodate)

A. PASSPORT REQUIREMENTS FOR FIRST TIME APPLICANTS

Personal Appearance is required.

Duly accomplished application form.

Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO.

Three (3) colored photos of the applicant taken against a royal blue background. Applicant should be in decent attire with collar. Photo must be of good quality, and must have been taken within the last six (6) months. Photo size: 4.5 cm x 3.5 cm. Facial image size: Not less than 3 cm. DFA has the right to reject photos that do not comply with specifications and international standards.

B. MEDICAL

What should I bring to my medical examination?

Appointment letter

Appropriate medical examination fees ($182.50)

Passport (valid for at least 6 months)

Two colored pictures (2 inches x 2 inches) with a white background

C. CFO

Counseling Requirements for Filipino Spouses and Other Partners of Foreign Nationals

Two (2) valid identification cards (IDs) with photograph;

Duly completed guidance and counseling form;

If married, certified true copy and photocopy of marriage contract on security paper from the National Statistics Office (NSO), or Local Civil Registry Offices (LCRO); or original and photocopy of marriage contract duly authenticated by the Philippine Embassy/Consulate (if married abroad)

Other documents as may be required by the counselors; and

Payment of P250.00 counseling fee.

Note: Call those agencies before going in to be sure of the updated requirements.
