How do you fill-out this last part of the form?

Oath or Affirmation of Sponsor.

I acknowledge that I have read Part lll of the Instructions, Sponsor and Alien Liability, and am aware of my responsibilitiesas a sponsor under the Social Security Act, as amended, and the Food Stamp Act, as amended.

Signature of sponsor_________________ Subscribed and sworn to (affirmed) before me this _____________day of________, _________ at______?________. My commission expires on_______?________

Signature of Officer Administering Oath____________?______________ Title______?____

ok, the first four lines needs my signature, day of the month, month, then year.

how about this part?: "at" , "my commision expires on" , "Signature of Officer"..., "Title"

this part is filled out by the person who will notarize this form right?

All you do is sign, in the presence of the notary. The notary should fill out the rest.
