electronic form filing question

The guy who I paid to help me with the visa sent me this .

What I don't understand is it asks for a photo to be atached ,and whats the bar code thing all about,has someone done this form ,whats the deal ?

The online form will generate a printable form, you print that form and bring it to the embassy for interview, the online system does not keep any information.

The online form generates a unique scanable barcode on page 3 that contains all the data entered on pages 1 and 2.
