The guy who I paid to help me with the visa sent me this .

(This form is an electronic form that get's filled out online at https://evisaforms.state.gov/ This is the actual visa application form. We suggest you gather all the information first on a paper form to have the information complete and ready before filling out the electronic form. )

What I don't understand is it asks for a photo to be atached ,and whats the bar code thing all about, has someone done this form ,whats the deal ?

Thanks for any advice my fiancee loves this site now too, she is always reading imformation here

That's the DS156 form, that is filled out when you have NOA2 approval and is sent to your fiancee in her country. The photo gets attached at the embassy by their folks when you bring in the information (at least that's how they do it in Kyiv). The photo is simply stapled into the appropriate place.

You don't need to fill out the form with the bar code until your petition has been approved and it is needed for the interview.

Sorry, but I don't think this guy you hired knows what he's doing. If he's getting paid, why are you filling out your own forms? Was that what you agreed to?
