Proof of Citizenship

Copy of the Birth certificate (front and back) for the US Citizen or a copy of ALL pages of the US Citizen's passport issued with a validity of at least 5 years or a copy of the US Citizen's naturalization certificate (front and back). This is used to establish citizenship.

Does this mean that the passport needs to be valid for the next 5 years starting the date of submition of the I-219F? I have my U.S passport that was issued on 2001 and its expiring in 2011.

It means the passport must have been issued with at least five years validity..... not that you have to have at least five years remaining on it.
