This is caro06's fiance. Not sure if this is the right place to post this, but I was just getting

ready to mail this out today and I chickened out cos I am unsure about a few things that need to

be sent along with the affidavit as listed below.

Are most of these items below just things you are signing off to on the affidavit itself, or do you need to type up a separate letter with actual statements?

Such as in number 2 (Other obligations) What are they looking for exactly?

And 3 and 4 are they looking for specific documentation?

I guess let me know if you understand what I am asking for before I ask anymore questions.

Information regarding his or her annual income;

Where material, information regarding his or her other resources;

1. Obligations for the support of members of his or her own family and other persons, if any;

2. Other obligations and expenses;

3. Plans and arrangements made for the applicant's reception and support;

4. An expression of willingness to deposit a bond, if necessary, with the Immigration and Naturalization Service to guarantee that the applicant will not become a public charge in the United States; and

5. An acknowledgement that the sponsor is aware of his or her responsibilities under the Social Security Act, as amended, and the Food Stamp Act, as amended; that the affidavit will be binding upon to sponsor for three (3) years after entry of the named persons; and that the affidavit and supporting documentation may be made available to a public assistance agency. (The provisions of the above laws are contained in form DS-1858, Sponsor's Financial Responsibility Under the Social Security Act, and printed in Part III of the instructions for Form I-134).

6. The sponsor should include in the affidavit a statement concerning his or her status in the United States. If the sponsor is an American citizen the affidavit should include a statement about how United States citizenship was acquired. If naturalized, the affidavit should indicate the date of naturalization, the name and location of the court, and the number of the sponsor's certificate of naturalization. If the sponsor is an alien who has been lawfully admitted into the United States for permanent residence, he or she should state in the affidavit the date and place of admission for permanent residence and the alien registration number which appears on his or her Alien Registration Receipt Card

I also read on another website that the affidavit should be on one piece of paper front and back, not 2 serparate pages. I have not read anything about that here, so I was wondering if that needs to be done?

Here is what I have asembled for I-134.

Please let me know if I should have, or should be including anything else!

* Signed and notarized I-134

* Statement from bank officer confirming Date account was opened, Total amount deposited for the year, and present balance.

*Statement from my employer stating when I started the job, my slary paid, and that the job is permanent, and I am an employee in good standing. ( I make $47,000 per year and have no dependants)

* Copy of my 2006 tax return

* 6 pay stubs dating back to when our petition was received by USCIS up to the present time

* Copies of stock certificates I own ( I recently became a business owner in 2007. I have not yet done a tax return for 2007. Stock is worth approx. $38,000)

* Copy of my 401k earnings ( I don't think this is needed, but I wanted to send it for additional info)

Anything else I should be including? Any thoughts?
