EVOLUTION

Not to throw a fly in the ointment, but my wife was refused her visa until she got her second Td shot. The woman told her if she got it and returned by 1 PM she would issue her visa. It was 11 AM. We raced to Bumrungrad and got the shot and made it back in time. While we were there we talked to others having to get a shot and all were Cambodian. I haven't heard of any others having this problem. Maybe the lady at window 5 was having another bad day. :huh:

I am sure everything will go allright but just wanted to let you know what happened to us.

Hey thank you Sam for your input. My fiancée is done with the medical exam and got 3 vaccination done. She's not sure what they were tho. Sadly things didn't go as planned. She didn't get a chance to have those med forms copied before the nurse sealed the envelops. She was saying that all the envelops was sealed and on one envelop they have something written on the outside of it in English. I wonder what it is.

Anybody know what this form is for? I found it in the NIV checklist from the embassy website.

EVOLUTION,

I just helped my MIL and FIL fill out this form a couple of months back for their B2 visitor's visa. That form is for non-immigrant visa applicants. Its a requirement that they fill out that form along with the envelope purchased inside the US Consulate compound, so that the US Embassy can mail the applicant's Passport back to either the applicant's home address or the designated Post Office for pick up. I don't think K-1 applicants need to fill out the form.

Unless this is a new form, it is not applicable to the K1 visa.

The packet 4 is written confirmation of the interview appointment date and time. It also contains a checklist of what to bring to the interview.

I had noted earlier in the thread about this. If the envelope is sealed for the embassy, then the second series couldn't even be updated on the sheet. I guess they could open it. My Fiance was told that she could return for the second series. I think that is just for her benefit and no that of the visa. Your fiance probably got the same 3 my fiance got. They were MMR, Varicella and Td. I think the MMR and the varicella require a second series in one month.

I never recieve our packet 3, but I got our packet 4 today and express shipped to my fiancée earlier today.

Hey all, I have some information that may be useful. When Pao got her packet 3 from the US Embassy in Bangkok, we noticed that the forms were different than those posted on the embassy US Embassy website. The checklist is not included on the website, or the instructions in Thai, or the new requrement regarding sending the photographs or the passport photos in. Pao scanned both front and back of every document included in the actual packet received and I have posted them in TIF (high resolution scans) and in PDF format here:

:angry: Boy oh boy! appearantly one of the paperwork that my fiancee recieved wasn't her. The form DS3025 was someone else copy. Other than that everything was under her name. what a Shiiiznut.

Yeah Bill. And I am still anxiously waiting to see if our interview date will be there when they update. So....I sent an email to the embassy and asked them why it was not updated, after all, they did suggest I check there for our interview date. They responded and said the schedule on the website was updated MONTHLY. :rolleyes: We all know that is not true because we have all been looking at it for awhile. :whistle: I believe it was updated weekly, on Tuesday. Our government at work, the right hand not knowing what the left is doing.

Anybody know what to do about DS-230, Part II??

Packet 4 from the Embassy has a page titled "Apppointment Letter for Immigrant Visa Applicants" and on that page the following quote appears, "Please Read carefully the attached DS-230 Part II....... Fill Out this form completely....". My Fiance just got Packet 4 and this document is not in the packet. This document is not posted on the Embassy site. Also, I did not see this document in either of the Packet 4 posts from Tucson Bill and Evolution. This form also is not on the page titled "Order of Documents", which lies out all the documents to be presented for the interview.

Is this document simply a sworn statement to be completed at the Embassy? Does anyone have this? Should I get it, and if so, where?

Also, a note about Packet 4. I think it needs a facelift. It's like a hodge podge of paperwork. Two appointment letters, one is filled out. What's the other one for? This DS-230 weirdness (assuming they didn't just forget ours). I guess the packet is used for multiple visa types and they just throw in things as they need.

Also, one more quick question. Does the K1 require the DS-157? We are filling out the DS-156 and DS-156K, but not sure if the DS-157 is necessary. Again, confusion in the packet 4 "Order of Documents". It is not a K1 specific packet and the is an "or" in the list of documents. The DS-157 is posted on the Embassy site under K1 so I think so. Anyone know?
